Handoff documentation and traceability assessment
Choose one evidence state for each core handoff dimension. Every calculation remains in this browser page and describes only the structure of the information supplied.
What this measures: documentation coverage and provenance traceability only. It does not determine scientific validity, laboratory quality or capability, method or site equivalence, regulatory compliance, appropriate acceptance criteria, or readiness to execute.
Interpret the responses with the accountable study team. This scorecard records structural information only. It does not make a scientific decision and does not authorize a handoff.
Evidence-state reference
Use these deterministic states consistently. Presence and traceability are scored; scientific correctness is never scored.

A complete score does not prove that a study should proceed. It shows whether the supplied handoff packet is structurally reviewable and where qualified sponsor and laboratory reviewers still need to resolve gaps or conflicts.
Score the structure, never the science
The scorecard evaluates whether core declarations are present and whether each declaration identifies a source, responsible owner, and version or date. It does not inspect the truth, adequacy, scientific plausibility, or regulatory status of the supplied information.
A high structural score cannot establish that a design is rigorous, a laboratory is capable, a method is suitable or transferable, acceptance criteria are appropriate, or the study should proceed. Those remain separate decisions for qualified people working in the exact intended-use context.

Use four visible evidence states
Missing means no declaration was supplied. Declared means a value, explicit unknown, or planned decision was supplied but its source, owner, or version is incomplete. Traceable means those provenance references are present; it does not mean the declaration is verified or true.
Conflict means multiple supplied declarations disagree. A conflict counts as present for documentation coverage but never as traceable, and it remains visible even if the overall coverage percentage is high.

Review counts and flags together
The result shows numerator and denominator for coverage and traceability so a percentage cannot hide how few prompts were considered. The deterministic state is based on missing information, unresolved conflicts, and provenance gaps rather than arbitrary pass bands.
Documentation traceable for handoff review is the strongest possible output. It means only that the structural packet can be reviewed by people; it must never be shortened to ready, pass, validated, compliant, or ready to execute.
